Training: Four (4) hours of relevant training
Experience: One (1) year of relevant experience
Eligibility: Security Guard License (MC No. 10, s. 2013 Category IV)
Place of Assignment: SSF, UPV, Iloilo City

1. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) and Work Experience Sheet (WES) which can be downloaded at csc.gov.ph;
2. Original authenticated copy of Transcript of Records and Diploma issued by the Registrar or Certificate of Highest Educational Attainment issued by authorized official of Department of Education;
3. Original authenticated copy of Security Guard License issued by the Philippine National Police;
4. Certificate of performance ratings obtained in the last two (2) rating periods (January to June 2022 and July to December 2022); Numerical and equivalent ratings must be indicated;
5. Certificate of attendance to trainings, seminars and workshops for the last five (5) years (March 2018 to February 2023);
6. Certificate of employment from previous employers (only for work experience relevant to the job);
7. Certificate of Accomplishments (Awards/Citations/Innovations/Critical Incidents)
