3. Professional License (RA 1080) issued by the PRC;
4. Certificate of employment issued by the HRMO or authorized officials (only for work experience relevant to the job);
5. Certificate of performance ratings obtained in the last two (2) rating periods (July to December 2021 and January to June 2022). Numerical and equivalent ratings must be indicated;
6. Certificate of attendance to trainings, seminars and workshops for the last five (5) years (1 January 2018 to 31 December 2022);
7. Certificate of Accomplishments (Awards/Citations/Innovations/Critical Incidents)
